**Required:** - Your tirzepatide powder vial - Bacteriostatic water (BAC water) -- this contains a preservative that keeps the solution safe for multiple uses over 28 days - A sterile syringe, 3mL or larger, with needle - Alcohol swabs - A clean, flat, well-lit workspace - Clean hands (wash thoroughly with soap) **Strongly recommended:** - A calculator or your phone's calculator app - A permanent marker for labeling - A notebook to log reconstitution date and concentration ## Reconstitution Calculations ### Understanding Concentrations **For a 30mg Tirzepatide Vial:** | BAC Water Added | Concentration | Per 0.5mL | |-----------------|---------------|-----------| | 3mL | 10mg/mL | 5mg | | 6mL | 5mg/mL | 2.5mg | | 10mL | 3mg/mL | 1.5mg | **For a 60mg Tirzepatide Vial:** | BAC Water Added | Concentration | Per 0.5mL | |-----------------|---------------|-----------| | 6mL | 10mg/mL | 5mg | | 12mL | 5mg/mL | 2.5mg | ### Choosing Your Concentration **Higher concentration (10mg/mL):** - Smaller injection volumes - Fewer injections per vial - Good for higher doses **Lower concentration (5mg/mL):** - Easier dose measurement - Better for lower doses - More precise titration ## Step-by-Step Reconstitution ### Preparation Phase **Step 1: Gather Materials** - All supplies within reach - Clean workspace - Good lighting - Hands washed thoroughly **Step 2: Inspect Vials** - Check expiration dates - Verify powder appearance (white/off-white) - Ensure BAC water is clear - No damage to seals **Step 3: Calculate Your Volume** - Determine desired concentration - Calculate BAC water needed - Double-check math - Write it down ### Mixing Phase **Step 4: Clean Vial Tops** - Wipe both vials with alcohol - Let dry completely (30 seconds) - Don't touch cleaned surfaces **Step 5: Draw Bacteriostatic Water** - Remove syringe cap - Insert needle into BAC water vial - Invert vial - Draw calculated amount - Remove air bubbles **Step 6: Add to Tirzepatide Vial** - Insert needle through center of stopper - Aim at side of vial (not directly on powder) - Inject slowly - Let water run down side **Step 7: Mix Gently** - Do NOT shake vigorously - Gently swirl in circular motion - Roll vial between palms - Continue until fully dissolved **Step 8: Inspect Solution** - Should be clear - No visible particles - No cloudiness - Slight yellow tint acceptable ### Post-Reconstitution **Step 9: Label the Vial** Write on vial: - Reconstitution date - Concentration (mg/mL) - Expiration (28-30 days) - Your initials **Step 10: Store Properly** - Refrigerate immediately - 36-46F (2-8C) - Protect from light - Keep upright ## Dosing from Reconstituted Vial ### Dose Calculation Formula ``` Volume to inject = Desired dose (mg) Concentration (mg/mL) ``` ### Example Calculations **At 10mg/mL concentration:** | Desired Dose | Volume to Inject | |--------------|------------------| | 2.5mg | 0.25mL | | 5mg | 0.5mL | | 7.5mg | 0.75mL | | 10mg | 1.0mL | | 15mg | 1.5mL | **At 5mg/mL concentration:** | Desired Dose | Volume to Inject | |--------------|------------------| | 2.5mg | 0.5mL | | 5mg | 1.0mL | | 7.5mg | 1.5mL | | 10mg | 2.0mL | ## Storage After Reconstitution ### Refrigerated Storage - Temperature: 36-46F (2-8C) - Duration: 28-30 days maximum - Keep in original vial - Protect from light ### What to Avoid - Freezing (destroys medication) - Room temperature storage - Direct sunlight - Excessive handling ## Troubleshooting ### Powder Won't Dissolve **Possible causes:** - Water too cold - Not enough mixing time - Damaged product **Solutions:** - Let BAC water reach room temp - Continue gentle swirling (up to 30 min) - If still undissolved, contact pharmacy ### Solution Is Cloudy **Possible causes:** - Contamination - Degradation - Improper storage **Action:** - Do not use - Discard safely - Contact pharmacy for replacement ### Particles Visible **What to do:** - Gently swirl again - If particles persist, do not use - Some settling may be normal - When in doubt, replace ## Best Practices ### Do's Work in clean environment Double-check calculations Use fresh needle each time Store properly after each use Keep reconstitution log Follow expiration dates ### Don'ts Shake vigorously Use if cloudy or particulate Store at room temperature Reuse needles Use past expiration Expose to heat/light ## Before Every Injection: A Quick Checklist Each time you draw from the vial, take 30 seconds to verify: 1
