For patients who are tolerating the maximal therapeutic dose of a once-daily or twice-daily GLP-1 receptor agonist (exenatide 10 g twice daily, liraglutide 1.8 mg once daily, or lixisenatide 20 mg once daily) but who are changing to a once-weekly GLP-1 receptor agonist, we recommend starting dulaglutide once weekly or exenatide once weekly at the maximal therapeutic dose (dulaglutide 1.5 mg, exenatide 2 mg)
